Your body is like a carefully balanced water system, with fluids moving in and out every day to keep everything running smoothly. But when this balance tips too far in either direction, the consequences can range from uncomfortable to life-threatening. Understanding how dehydration and oedema develop helps us recognize these conditions early and respond effectively.

When the body runs dry: understanding dehydration
Imagine your body as a complex network of rivers and streams, with water flowing to every cell and tissue. When you lose more fluid than you take in, this network begins to dry up. Dehydration occurs when the body uses or loses more fluid than it takes in, leaving it unable to carry out its normal functions.
The causes of dehydration are more varied than most people realize. While not drinking enough water is obvious, illness often triggers severe fluid loss. Diarrhea and vomiting can cause dramatic losses of both water and essential minerals called electrolytes in a very short time. A high fever increases fluid loss through sweating, and when combined with diarrhea or vomiting, the situation becomes even more dangerous. Even something as simple as a cold or sore throat can lead to dehydration if it makes you less likely to eat or drink.
Hot weather and physical activity create another pathway to dehydration. When you sweat heavily during exercise or on a humid day, your body loses significant amounts of fluid. Athletes in endurance events face particular risk, especially if they don’t replace fluids consistently during their activity.
Recognizing the warning signs
Your body sends clear signals when it needs water. Thirst is the most obvious, but by the time you feel thirsty, dehydration has already begun. Other symptoms include dark-colored urine, urinating less frequently, tiredness, dizziness, and confusion. In children, watch for fewer wet diapers, no tears when crying, and sunken eyes or cheeks.
Severe dehydration becomes a medical emergency. When fluid loss reaches extreme levels, blood pressure drops dangerously low, leading to dizziness and fainting. The heart races as it tries to compensate for reduced blood volume. Confusion sets in as the brain is deprived of adequate fluid. In the most severe cases, dehydration can cause seizures, kidney failure, and even death. Research shows that a water loss of 20 percent can be fatal.
When fluid accumulates: the problem of oedema
While dehydration involves too little fluid, oedema represents the opposite problem. Oedema is swelling caused by fluid trapped in your body’s tissues, most commonly affecting the feet, ankles, and legs. The swelling occurs when fluid that normally circulates in your blood vessels leaks out and accumulates in the surrounding tissues.
Several mechanisms can trigger this fluid accumulation. Sometimes it’s as simple as gravity pulling fluid down when you sit or stand for long periods. But more serious causes involve disruptions to the body’s fluid balance systems. When the heart weakens and cannot pump blood effectively, pressure builds up in the veins, forcing fluid into tissues. Kidney disease prevents the body from eliminating enough sodium and water, creating pressure that pushes fluid out of blood vessels.
The protein connection
One particularly important cause of oedema involves low protein levels in the blood. Proteins, especially albumin, help keep fluid inside blood vessels through osmotic pressure. When there’s a lack of protein albumin in the blood, fluid can leak out of blood vessels more easily. This protein deficiency can result from severe malnutrition, as seen in the condition kwashiorkor, where children consuming insufficient protein develop characteristic swelling.
Kidney disorders can also deplete blood proteins. In nephrotic syndrome, damaged kidneys allow too much protein to escape into the urine, lowering blood protein levels and triggering widespread oedema. Similarly, liver disease reduces the production of proteins and increases pressure in blood vessels, leading to fluid accumulation in the abdomen and legs.
Managing oedema effectively
Treatment for oedema depends on its underlying cause. When low protein levels are responsible, increasing dietary protein intake can help restore balance. For mild cases related to prolonged sitting or standing, simple measures like elevating the legs and moving regularly can reduce swelling. More severe cases may require diuretic medications, which help the kidneys eliminate excess sodium and water from the body. Reducing salt intake is crucial, as salt causes your body to retain water, which can leak into tissues and cause swelling.
The lifesaving simplicity of oral rehydration therapy
When severe diarrhea strikes, particularly in children, rapid dehydration can quickly become life-threatening. For decades, the standard treatment required intravenous fluids administered in hospitals, putting lifesaving care out of reach for millions in resource-limited settings. Then came a breakthrough so simple it seemed almost too good to be true: oral rehydration therapy.
ORT consists of nothing more than water, salt, and sugar in specific proportions. The medical journal The Lancet called it “the most important medical advance of the 20th century.” It is estimated that oral rehydration therapy has saved around 70 million lives since its introduction in the late 1970s. This remarkable success comes from ORT’s ability to treat one of the deadliest aspects of diarrheal disease: dehydration.
How a simple solution works wonders
The science behind ORT is elegant. The cells lining your gut have special receptors that actively absorb sugar molecules, but this process requires sodium to work effectively. When both sugar and sodium are present together in the right amounts, they enhance each other’s absorption. As these molecules move into the gut cells, water and other electrolytes follow along. This mechanism allows the body to absorb fluids even while losing them through diarrhea.
The World Health Organization recommends a specific formulation containing precise amounts of glucose, sodium chloride, potassium chloride, and trisodium citrate mixed with clean water. Pre-packaged oral rehydration salts are available, or a simple homemade version can be prepared by mixing eight level teaspoons of sugar and one level teaspoon of salt in one liter of clean water.
A global health success story
The impact of ORT has been staggering. Since the World Health Organization began using ORT in 1978, the annual death rate among children under 5 suffering from acute diarrhea has fallen from 5 million to fewer than 1 million. ORT costs only about 50 cents per treatment course in lower-income countries, making it accessible even in the poorest communities.
What makes ORT particularly valuable is its effectiveness for mild to moderate dehydration without requiring hospitalization or special equipment. Parents can administer it at home at the first signs of illness. For children who are vomiting, giving small amounts frequently, even just 5 milliliters every five minutes, allows successful rehydration without overwhelming the stomach.
